How donations are used

Diabetes is a well-known disease, but it is not well known in Japan that there are different types of diabetes, such as Type 1 and Type 2 diabetes. Most diabetics have “Type 2 diabetes,” for which diet and exercise are the basis of treatment; Type 1 diabetes accounts for only a few percent of all diabetics in Japan, and it often occurs even in children. Once developed, it is an incurable disease requiring insulin therapy of 4-5 injections daily for the rest of one’s life.
The funds will be used to support research to make Type 1 diabetes a “curable disease” and support activities for Type 1 diabetes patients with various needs, from children to the elderly.

I am very motivated to be part of the Tokyo Marathon Charity team and support IDDM Network because in my life I never let Diabetes define who I am, I always lived it as an obstacle to overcome but not a limit.
I believe and proven many times that with the right training, strategy, therapy, nutrition and technologies we shall not give up on our dreams and we can achieve incredible results.

I realized it is not the same for many living with T1D, who have fear of travelling and engaging in endurance sports because of possible risks we may have. That’s why, since 2023, I exposed myself on social media (instagram profile @simone.carniglia): to inspire and motivate other T1Ds to follow their dreams, but also to raise awareness on T1D, that is a really underestimated and little known condition by the general public. Usually T1D is wrongly associated with lifestyle and nutrition, while actually it is an auto-immune disease at present with no cure, just a life saving therapy.
I love Japan, I have already visited it a few times and always love to come back, I have run the Tokyo Marathon once before in 2023. Now I am running all the 6 Majors for a 2nd time to become one of the first T1Ds to ever achieve so (if not the very first) and I am just missing Tokyo and Chicago and I will both run them in 2026. I chose to run Tokyo for charity to support IDDM Network work in japan to inspire, raise awareness and collect funds to improve the life of Japanese T1Ds and possibly find a cure.
Another goal of mine is to make races more inclusive for T1Ds, because we are not considered as Paralympic athletes, but at the same time face many more invisible challenges and additional burdens. Just a few changes in races regulations would make our experience less challenging and safer for us. In 2024 I managed to obtain this from the New York Marathon and I would like them to be implemented in all races.

My name is Simone Carniglia. I am 38 years old, born and raised in Italy, where I still live and train. I am a full time Mechanical Engineer and Project Manager for a multinational company in the energy sector.
I was diagnosed with Type 1 Diabetes. I started running from zero and very overweight just 8 years ago after a personal low in 2018, and since then I’ve run 16 Marathons (and counting), of which 10 world marathon majors, and achieved incredible feats, including a Guinness World Record in the 6 World Marathon Majors as a T1D. Since January 2025 I am also an Asics Ambassador. In August 2025 I will run Sydney Marathon to become the first T1D to achieve that and to extend my Guinness to the newly established 7 Majors.

STEP2. Apply to make a donation through the organization’s donation system.

Field Size 40
Eligibility Must be at least 19 years of age on the Tokyo Marathon 2026 race day.
Must be able to complete the marathon within 6 hours and 30 minutes.
Charity Runner Selection Criteria 1. Donations per person is 100,000 yen or more
2. Selected based on the required content of the “Thoughts on charity & Messages” entered at the time of application.
※The results of runner selection will be announced from Tuesday, July 15 to Friday, July 18.(JST)
Donor program 1. Finish lounge exclusively for Charity Runners will be made available. (provisional)
2. Charity T-shirt will be distributed.
Others * Charity Runner registration (participation) is voluntary for donators.
